Structured records review

If you have prior imaging, endoscopy reports, labs, or a bariatric history, sending them ahead lets our clinical team review your context before any conversation.

What to include

  • Recent labs (CBC, metabolic panel, lipid panel, HbA1c).
  • Upper endoscopy report, if you have had one.
  • Abdominal imaging (ultrasound, CT, MRI), if available.
  • Current medication list, including any GLP-1 therapy.
  • Summary of prior bariatric procedures, if any.

How to send them securely

Email records to records@endoscopicsleeveguide.com. Do not include full identifiers you are not comfortable sharing. For encrypted upload, our coordinator can send a secure link after you contact us via the evaluation form.

What we can and can't do with records

Records help contextualize an educational conversation and help a physician prepare for a formal evaluation. Records review by itself is not a diagnosis, not a treatment plan, and does not create a physician-patient relationship.
